Nursing Grand Rounds – Upper Valley Community & Parish Nursing: Enhancing the Continuum of Care

The presence of Community/Parish (CN/PN) nurses in our region is an under-utilized resource.  CN/PN nurses provide services that can enhance community-based care options - especially for frail older adults. This presentation will provide clarification of the differences between “skilled” home care services and the services provided by the CN/PNs. We will also discuss how CN/PNs provide a resource that complements the work of the skilled home care agencies and the process for initiating an appropriate referral.

Learning Outcome
