Nursing Grand Rounds – Older Adult Falls Prevention: DHMC and Local Community, Statewide and National Initiatives

This presentation will discuss current data related to older adult falls and what we can all do as a team to prevent falls. Discussion will include programs and efforts taking place here at DHMC and the local community as well as statewide and national efforts. We will discuss concrete ways to incorporate falls prevention screening into practice and how and where to refer patients to evidence based programs proven to reduce fall risk.

Learning Outcome

At the conclusion of this learning activity, participants will be able to describe at least one evidence based fall prevention initiative taking place at DHMC, in the local community, statewide and nationally including how these efforts can be incorporated into practice.
