Emerging Focus on Age Friendly Health Systems: A Step Forward in Falls Prevention

Join us for our annual program focusing on Falls Prevention efforts at the local and national level. As hospitals and health systems prepare to care for our aging population, many are using the Age-Friendly Health Systems initiative to provide care focused around the 4Ms- What Matters Most, Medication, Mentation, and Mobility. Learn how these are being implemented in New Hampshire and beyond, and can increase patient safety, improve outcomes, increase patient and family engagement, reduce harm from falls, and decrease hospitalizations. We will learn practical and innovative strategies for implementation. While there is a special focus on Mobility, we will address how all of the 4Ms interconnect.

At the conclusion of this learning activity, participants will be able to discuss the process of implementing specific quality improvement projects to become an Age Friendly Health System including the benefits to both patients and the system itself.

The follow steps must be completed in order to receive credit for this event:

  • Log onto https://ce.dartmouth-hitchcock.org/login.aspx
  • Click on the My Activities link in the left side menu. Then click on the name of the activity you attended.
  • Click on the Resume button.
  • Click the Continue button until the Evaluation tab is active, and then click the Launch button.
  • Fill out the evaluation and then click submit. Credits will then be applied to your transcript. You can view your updated transcript by clicking on the "View Transcript" button.
