Serious Illness Conversations, Part 1: Framework and Communication Practice

This 3 hour interactive, skills-based session gives clinicians of any discipline a talking map for effective, efficient goals of care conversations.

Led by DH Serious Illness Care Program faculty, the session features active learning and reflection, including progressive skills practice. Participants will leave the session with the skills and tools to begin having these conversations with their patients.

To maximize time for skills practice, prior to the session participants will be expected to: watch a short video (8 minutes) and to print out training materials.

The faculty for this session is Dr. Max Vergo.

Training sessions are virtual during the COVID pandemic and remain interactive and high energy!

In order to participate and get credit for this activity, you will be required to have working audio and visual. If you don’t, the facilitator will ask you to leave the activity and reschedule when your technology is functional.

Learning Outcome(s)

At the conclusion of this learning activity, participants will be able to use the Serious Illness Conversation Guide (SICG) to enhance their competence in making value-aligned treatment recommendations and documentation for care of seriously ill patients.


Dartmouth-Hitchcock is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Dartmouth-Hitchcock designates this live activity for a maximum of 3 AMA PRA Category 1 Credits™. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

Receiving Credit

The follow steps must be completed in order to receive credit for this event:

  • Register for the event online
  • Attend the event or session(s). A credit tracking sheet will be provided.
  • Complete the online evaluation - Must be submitted within 30 days after the activity

    Accessing the Online Evaluation (The evaluation is available the day after the event)

  • Log onto https://ce.dartmouth-hitchcock.org/login.aspx
  • Click on the My Activities link in the left side menu. Then click on the name of the activity you attended.
  • Click on the Resume button.
  • Click the Continue button until the Evaluation tab is active, and then click the Launch button.
  • Fill out the evaluation and then click submit. Credits will then be applied to your transcript. You can view your updated transcript by clicking on the "View Transcript" button.
